Abstract: The objectives of this study were: 1- to assess gene expression by quantitative PCR of osteoclastogenesisrelated factors, receptor activator of NF-_B ligand (RANKL) and osteoprotegerin (OPG), in soft tissues exhibiting different peri-implant clinical conditions; 2- to evaluate the total amounts of RANKL and OPG in peri-implant crevicular fluid (PICF) in sites exhibiting different clinical conditions before and after mechanical anti-infective therapies. Thirty-five subjects and a total of 40 implants, including 10 healthy (H), 10 with mucositis (MUC) and 20 with peri-implantitis (PER) were included in this study. MUC was treated with abrasive sodium carbonate air-powder and teflon curettes and hyperplasic tissue removal while PER was treated with open surgical debridement using abrasive sodium carbonate air-powder and teflon curettes. Peri-implant soft tissue biopsies were harvested from these 3 clinical profiles to detect the mRNA levels for RANKL and OPG, using quantitative PCR. PICF were collected at baseline and at 3 months post-therapies to evaluate the total amount of RANKL and OPG by ELISA. Visible plaque accumulation (PI), mucosal bleeding (MB), bleeding on probing (BOP), suppuration (SUP), probing depth (PD) and relative clinical attachment level were also assessed at baseline and at 3 months after therapies. The anti-infective treatments resulted in a significant improvement in all clinical parameters (p<0.05). The highest gene expression of OPG/RANKL ratio was observed in H and decreased as the peri-implant disease severity increased (p<0.05). Moreover, the total amounts of OPG and OPG/RANKL ratio were higher for H than for untreated PER (p<0.05) and, this difference was not observed after the anti-infective mechanical (p>0.05). Therefore, this study demonstrated that the osteoclastogenesis-related factors play role in different periimplant clinical conditions and the anti-infective therapy may locally modulate the OPG/RANKL ratio in periimplantitis
