Abstract: This prospective in vivo study has as main objective to evaluate the structural and biological performance of single-tooth implant supported-zirconia restoration in aesthetic region. Sixteen subjects were selected for the placement of thirty-one implants in aesthetic region. For the sample group fifteen single-tooth implants were fabricated. Zirconia abutments (YTZP) were used followed by the addition of feldspatic porcelain (metal-free implant supported restoration) in experimental group. For the control group, sixteen implants supported restoration were made with metallic abutments followed by the application of porcelain (metal-ceramic implant supported restoration). Both groups were screwed on the Implants, being therefore removable, and also submitted to chewing. For the biological evaluation bone level (BL) was compared through X-ray imaging on the baseline, thirty, and seventy-five days after the procedure. The structural performance in the experimental group was evaluated before, and after the porcelain addition at baseline, thirty, and seventy-five days, through analysis of epoxy-resin replicas by scanning electron microscope. The survival rate of implant supported-zirconia restoration was 93% and 100% for implant supportedmetallic restoration. One zirconia abutment was fractured during the reinstallation by the thirty-day. No chipping of the other veneering ceramic occurred at the both groups. The Mann Whitney tests showed no significant differences for the evaluation of BL at 75 days; however there was more bone lost at 30 days at implants supported-zirconia restoration (ρ<0.05). During the clinical evaluation, no soft tissue demonstrates problem for the aesthetic observation and no biological complication was found for the both groups. The zirconia group demonstrated the highest bone lost at 30 days. Soft tissues did not demonstrate clinical sings of inflammation or other clinical alteration. The method used for replicas construction was not adequate for observation of small details in the external hexagon. It is necessary more comparative long-term reports comparing zirconia and metallic abutments in large patient populations. The implants supported restorations demonstrated a good biologic and structured survival rate during a short clinical trial evaluation.
